Ohio sleevers - insurance question
My husband has BCBS Anthem (I do not) and his policy specifically excludes weight loss surgery.
But it is my understanding that other BCBS Anthem policies do cover it. It depends on whether or not your employer chose a plan that included weight loss surgery or not. If I remember correctly, the small business plans do not cover the surgery. I'm not sure, however, how many employees must be covered for a plan to qualify as a small business plan though.
