It's the sugar alcohols used to sweeten items that are sugar free.
You need to read labels VERY closely and know how your body will react to certain ingredients. Some folks become lactose intolerant, some don't. Some can't tolerate sugar alcohols after surgery while other don't. And some folks were lactose intolerate and/or unable to handle sugar alcohols before surgery.
If you haven't had issues with sugar alcohols prior to surgery, proceed with caution. It may be fine but check it out with a SMALL portion and see how your body reacts.

I'm thinking you are right and that you are finding out, pre-op, you need to steer clear of sugar alcohols. If it's making you super gassy now, it would be far worse immediately following surgery if you are full of the gas for lap surgery.
