5 year weight history question

So i had my last visit with the surgeon today (had to do the 6 months with a nut)  and they only have 4 years worth of weight history showing me at a bmi of 35 or greater.  We are missing 2008.  I only went to the Dr. once or twice right before we moved away from Florida.  I have called the Dr's office multiple times, The Surgeons office called multiple times.  We have been calling since JULY!!!!    We still have not gotten the medical notes.  I don't know what else to do. I am desperate for those notes for 2008!  Unfortunetly i can't afford to jump on a plane and fly to florida to personally request these notes. 

Anyone have any suggestions?  All my other papers are in order and they can submit for approval.

I had a similar issue, I was missing 2008 and 2009. I was laid off during that time and didn't have health insurance. My surgeon's office simply had me write a letter stating why those two years were missing and my insurance company never even blinked. Hopefully things will work out just as well for you. Good luck!

Before I realized I had crap coverage- 50% after 6000 deductible and a 6 month diet plan, 5 years of weight history and still possible denial. I can't believe I work for a fortune 50 company with that "insurance".

Anyway, I used my well woman exams instead of PCP. Is this your PCP or OB/GYN. Did you see any other docs in 2008?

Did you see any other dr in 2008 - maybe a gyno?  Maybe a note from someone else...
