Anyone willing to share PCP or personal letters that worked with insurers?

I am just starting the process. I have a supportive PCP but he doesn't have much experience recommending for my surgery. I expect a fight with my insurance company. I don't know anyone who has been approved by them. I am trying to be prepared as possible. Is anyone willing to share the letter they or their PCP sent that seemed effective at convincing the insurer to approve? I don't want to copy it but I am interested in getting a sense of how you make your case. Thanks Brenda
