How long did the approval process with Kaiser take?

From the time your PCP sent in a referal to your surgery date? Was the amount of time acceptable? Why do you think it takes Kaiser a great deal more time to approve patients. Does Kaiser delay approval in hopes patients will not follow through? What do you think?
