Anyone know what criteria BCBS Preferred Plus uses

I've checked with @ different centers for WLS. One said I needed 6 mos of medically managed weight loss and it didn't matter that my BMI was 39 because I had hypertension. The other one said my insurance required a BMI of 40 and it didn't matter even if I had co-morbidities. I'm confused, I called my insurance and they said that everyone is evaluated individually and my PCP would have to call and see if I qualify. Anyone out there know what BCBS Preferred Plus through GM requires?
