Do you get to choose a provider for surgery and which proc. or is it based on ins. ?

I have been reading all the questions and answers, but just have to ask one of my own. I have not even asked if my insurance covers this type of surgery or not yet because I first want to decide which type of procedure I would like. Do I get this option, or is it just like other stuff and my PPO will only take certain doctors and certain procedures. The reason I ask is that I would rather pay for the insurance myself and get what I want. I am leaning towards the Duodenal Switch which is not performed in any states even near me, so will I need insurance from an agent in that state? Hopefully you get what I am asking, I'm definitely not being very articulate right now. *g* To sum it up, if I want to use a doctor in another state for a procedure not done in my state (Arkansas), how do I do that. THERE, I GOT IT OUT!
