Cigna / Intracorp - Who does what & how do you get an approval??

I got a denial letter from Cigna PPO. They tell me to appeal it with Intracorp. Does anyone know how long this process usually take?? My surgery date is 3 weeks from now & I still am not approved......Can anyone help or make any suggestions?? Please
