Phone # for Kaiser So. San Fran Bareatric Dept. ?

A few months ago I posted a question in regards to Kaiser SSF. Several very helpfull people responded and provided me with the phone number to that office but due to some email issues I have lost the info. If anyone has the number please let me know. My paperwork was sent in to them 5 weeks ago and I was told I would have a response in 4-6 weeks so I wanted to call them and follow up. Thanks, Melissa
