I have an incisional hernia & wonder how to get panniculectomy approved.

I had RNY 5/23/00 and now have an incisional hernia that needs repaired. I understand that some insurance companies will approve panniculectomy if you're having a hernia repaired. Anyone with Empire familiar with this? P.S. My RNY was approved after a six week wait.
