Health America/Assurance and Post-op Plastic Surgery. . . .

I have HA of PA and wanted to know what I should be documenting inorder to get my excess skin removal paid for? I did go to the doctor or at least call the doctors last summer EVERYTIME I got a rash under my stomach. I HATE the thought of going through this again this summer. My arms are terrible too!!! Any help will be well appreciated.
