Has anyone used Dr. Cook or is familiar with the types of surgery he does.

No personal experience here, but I did want to lead you to Dr. Cook's page on this site. It has helpful information for insurance companies that have paid for surgery by him, member comments with links to their profiles (so you can see how they are doing)and e-mail addresses. Maybe you could e-mail them your specific questions? His page is on the "find a doc" link. He was not located on the "NC" page but I found him by typing in a "c".
