Question:
Has anyone with UHC chioce plan every got approved for gastric bypass in Ohio?

I have UHC Choice PPO in Tennessee. I did get approved...but it took 4
months and lots of phone calls. Essentially, UHC "lost" my file,
found it, tried to tell me that my surgeon didn't send it to the right
place, then told me, no its in the right place...back and forth. Finally
they had no choice but to approve me. My policy states I need only be
Morbidly Obease and nothing else, and of course I am. I am having surgery
on November 11th.

I am in Ohio and have United Healthcare Options PPO. I was approved with
no problem. I don't know what the differences between the Choice HMO and
Options PPO plans are. The relevant clause in my policy reads: "No
medical benefits shall be payable under any part of this Plan with respect
to: Surgery, treatment, programs, or supplies that are intended to result
in weight reduction, regardless of diagnosis, except covered services to
treat morbid obesity." The "except" part is what I was
covered under. I think coverage can vary depending on your employer.
