denial letter

i got a denial letter from my insurance today saying i did not meet the guidelines regarding support group and exercise counseling.....can anyone give me a good appeal letter....there isnt a support group in the lewisburg area...and i've been going to a health club...does that consitute exercise counseling?....i cant afford a personal trainner...so i'm at a loss....thank you
