If you just put it on hold just call them, if it's under a year I think a hold can be removed by calling and asking to reinstate. If you completely withdrew then yes I think your doctor needs to re-refer.

Under a year, you should just be able to call them and continue from where you were... if you put yourself on hold vs withdrawing. Give them a call. They'll help you out!
