Insurance usually pays only for a panniculectomy (removal of excess skin below the belly button) if it covers any plastic surgery at all. The usual requirements are that the skin hangs down to a certain point (usually below the pubic bone) and that you have documentation of a certain number of months of rashes or other skin issues and various prescription and non-prescription attempts to treat those problems.
Occasionally, people are able to get a full tummy tuck (with muscle tightening) paid for by insurance if they have other issues like a hernia that needs repaired, but it is considered cosmetic rather than medically necessary.
