How do you list your surgery and surgeon

Can someone please help...I have been trying to list my surgery and evertime I do it goes to a white screen    I then fill in the info and then get directed to another page that same can't be found...any help is appreciated...

Go to "My Account" and then "Surgery History". Choose "Add Surgery". Easy peasy!

thanks...thats what I do and then I fill all the info out click save and then it sends me to another page that says page can't be found...

Ditto.....I've been trying to add my surgery date for a while and get the same results you do. For a brief second I see one form when I click the "Add Surgery" link and then it switches to another form that when I put all the info. in it says page can't be found....I've put the info. in about 5x's and got the same result.  What is it Einstein said, "Stupidity is repeatedly doing the same thing and expecting a differernt result."  Anyway, I am having th same issue you are.
