Wow!!!! These insurance companies are very tricky!!! They finally disclose what consults and documentation they need in order to approve a MEDICALLY necessary insurance after I blow up thier phone and harrass them. Seriously....at least 5 phone calls before I got the info needed.

In benefit handbooks and even if you call they will give you very generic information like:

BMI>40 or >35 with comorbidities
Psych testing
Nutrition Eval
Physician supervised diet....

Ok so what they won't tell you:

You need to be cleared by a pulmonologist AND a cardiologist
You need to have a TSH drawn 
You need to have an H-pylori drawn....
yep....you gotta do this BEFORE they will even consider your surgery.

And your doctor's office needs to know this because they need to make the referals to all the other providers....so if you're young and have no cardiac history and have been cleared by everyone....insurance DOES NOT CARE what your surgeon thinks they want to pay for you to see a cardiologist...not that he is required to do any testing....but basically ask about cardiac symptomology and sign his name.

P.S. Secondary insurance runs out in 22 days. Somehow....I got into a cardiologist tomorrow!!!!! Wish me luck!
