November 19: Surgeon - weigh in with nurse, diet discussion, etc
December 17: Surgeon - weigh in with nurse, diet discussion, etc
January 3:        Pre-surgical clearance with primary care doctor
January 4:        Surgeon - weigh in with nurse, diet discussion / psych clearance / dietician on post-op diet
January ??:      Hospital pre-op testing
January 20:      Surgery!!
