April 30, 2010 - First Surgery Consult with Dr. Juliet LeeMay 27, 2010 - October 30, 2010 - Nutritional Consultantations with Claire LeBrun
May 4, 2011 - Psychiatric Evaluation with Dr. Ruby
May 5, 2011 - Psych eval submitted to Insurance Coordinator
May 6, 2011 - Paperwork submitted to Dr. Lee (surgeon) for approval
May 9, 2011 - Paperwork returned because several medical records were missing
May 10, 2011 - Schedule appt with PCP to complete records; blood test ordered
May 13, 2011 - Blood test taken
May 18, 2011 - Pre-OP physical with PCP
May 18, 2011 - Followed up with Insurance Coordinator to see if all records were completed. The EKG results are missing.
May 18, 2011 - Contacted PCP to make sure EKG results are entered into the computer.
May 19, 2011 - Contract insurance coordinator to see if anything else is missing
May 20, 2011 - Insurance coordinator confirmed all test results are accounted for in file
May 25, 2011 - Received medical clearance from PCP and all test are fine.
June 6, 2011 - Pre-Op appointment with surgeon
June 9, 2011 - Started pre-op liquid diet
July 1, 2011 - Pre-existing condition period ends with short term disability insurance carrier
