"11-18-02 Psych Exam with Dr. (Robert) Eric Roth in Johnson City. Quite a thorough discussion to determine whether or not I was ready for life-altering surgery and success. He had a page (front and back) of prepared questions that he asked me, with about 2 inches of space where he wrote notes for his report. Then I completed the 567-question MMPI. I was very pleased with the method he used and the things he asked me. I felt like he was only interested in what was best for me. "
