Kendra - I too have Cigna PPO and I got an approval. It can be done. What you need to do is start a letter that addresses your condition first. Have the correct diagnostic codes and treatments to start it off. Get a detailed history of your immediate family and what problems they may have, i.e., high blood pressure, heart disease, diabetes, obesity etc. Do a bulleted chart on all of the weightloss programs and products you've tried over the years. Finally, let them know that you have researched this surgery and your reasons for having it are as follows... be completely level-headed about it. If I could do this, you can too. If you want to see my letter and use it for a reference, please let me know. Once you have your letter completed get it together with all of your medical records. Get it all in to the Appeals coordinator and let go of it. If you truly have sound reasons for the surgery, they will approve you. At least, that was my experience. Good luck... Kendra, You had the psych exam and the surgeon supposedly faxed it to Cigna. So why does Cigna still need it? For the nutritional evaluation, you need to see a nutritionist and get clearance from them. You need to tell your surgeon's office that you want to see exactly what has been sent to Cigna. Was it faxed or mailed. I was told faxing was the best. Be persistent, but nice. I will pray for you during this trying time. pamAbout Me
