Why Insurance companies place obstacles

I am more than 200 lbs overweight with a BMI of 53, last time I checked, have co-morbidities of hypertension, obstructive sleep apnea, palpations with arrthymias, shortness of breath with minimal exertion, and have a diagnosis of depression with treatment consisting of 2 different antidepressants. What else does CIGNA want? They want 6 months of physician supervised weight loss program even though my secondary problems could potentially worsen over that 6 month period. What do they care that would be one less claim they would have to deny or put off...
