First official setback

I found out last Friday that in order for my insurance to approve my surgery, I needed all the chart notes from my doctor for each of the months. That is fine, so I call my PCP today and they inform me that there are only three months in my chart not six. I am SOOOO mad about this. How can you lose three months worth of my chart?? I am hoping that they will find the sheets and I won't have to worry about it anymore, but if they don't find it do I have to start my diet over again? I'm just not sure what is going to happen around this problem.
