2-28-07 - Mailed all medical records, family physician recommendation letter, weight assessment from Medical Mutual's website for myself (stating that I'm at high risk for many health conditions because I need to LOSE WEIGHT), and a personal 3 page letter to give the insurance review board a more personal look into my file and into my life.
3-1-07 - File sent to Medical Mutual for approval.
3-9-07 - Called Medical Mutual to make sure file was received - Not received, was told to have it re-sent.
3-13-07 - File re-sent to Medical Mutual
3-19-07 - Called to double check it was received - I'm APPROVED already!!! I received approval on 3-16-07.
3-21-07 - Attended weight loss seminar in Dayton.
3-23-07 - Received letter of approval from Medical Mutual for the Lap Band procedure.
3-23-07 - Unsubscribed from OH website and re-registered with a new username - this new name fits better :) Spent the rest of the evening working on my profile. I didn't have a profile before. This is really cool!
3-26-07 - Sent letter of change to insurance for change in procedure and surgeon - and they APPROVED the changes - I'M GONNA BE A SLEEVESTER :)
4-20-07 - Appointment with surgeon - hoping to get a surgery date!
4-30-07 - Pre-Op - DONE!
5-9-07 - SURGERY DATE!!
11-9-07 - 6 months post op - minus 98 pounds - WOW!
