Day 0 (07-02-07) Went to the Atlanta Medical Center Seminar. Was not impressed, so I also went to the Emory Seminar.
Seems both are very similar. AMC is Bariatric Excellence Center and Emory doctors have very good reputation.

My insurance requires:
BMI >=35 with co-morbidities.
3 months of physician supervised diet

Day 10-Got my clearance from LCSW. ALL CLEAR!
Day 16-Got my Body Composition Report ALL CLEAR! BMI = 35.15
               blood tests ALL CLEAR!
               Nutrition Counseling Form ALL CLEAR!
Day 17-Got my Sleep study done. ALL CLEAR!
Day 31-Got my clearance from Cardiologist 1. (Recommended going to cardio-thoracic surgeon)
Day 65-Got my clearance, history and necessity letter from my PCP. ALL CLEAR!
Day 67-Got my clearance from Cardio-Thoracic surgeon. ALL CLEAR!

Day 70-Mailed all paperwork to Atlanta Medical Center.
Day 73-Called AMC to check on paperwork. Left msg. Called again, got VM (twice). Sent email.
              Called again and Jackie O. said they received packet yesterday and it will now go to "Medical Review".
              Probably just to make sure my insurance covers it. I think I will call every Tues and Thurs to bug them.
Day 75-Called AMC and was told that a letter was sent about packet. Jackie would not tell me why.
Day 77-Got letter indicating 3 items were not submitted. The only one which I forgot to include was lab work.
Day 78-Got another letter indicating 3 different items were not submitted.
Day 80-Called AMC and spoke to Jackie to find out what the deal was. She said she did not "recognize" some letters as
                 clearances. Called PCP office to fax lab work to Jackie.
Day 82-Called AMC and they received fax of lab work. The paperwork is again in "Medical Review" at AMC.
             Jackie "said" she faxed paperwork to insurance company on Friday. We'll see. She says to call ins in 3-5 days.
            Plus she says the nurse wants copy of the CT Scan report. Don't know why since they have clearances from TWO cardios.

Day 112-Called insurance and found out it's been approved. The doctor put in a tentative date of 11-26.
Day 113-Got a call from AMC. They have everything scheduled.

Day 122-My first appointment with the doctor.
Day 129-Pre-op tests (x-ray, etc) and appt with a nutritionist. She says to plan on 1/2 day.

Day 137-(11/16) Surgery date. They originally had it set for 11-12, but for an outpatient clinic that was not on my insurance plan.
