From my understanding that is correct. Insurance companies have at max 30 days to respond. Hope that helps and good luck.

30 days unless they send you a letter stating that it is pending for further information.

I don't know if there are legal guidelines, but think it all depends on your insurance plan. I was very lucky: Harvard-Pilgrim of MA (which is rated as one of the best insurance plans in the country) approved me the day after my paperwork was submitted.
