Hi, I had straight Fl Medicaid and had to switch to STAYWELL. I dont know what area you are in but I used a Dr Richard Gordon in Clearwater area. Phone 727-586-3751. They entire staff is WONDERFUL!!! I did see other Dr's and chose Dr.Gordon. Call them and you can tell them I sent you. If he is not in your area ask the Medicaid office for a list of Dr's. Best of luck!

Go to the Florida Board on OH and look for the surgeon to takes your type on insurance in your area. Marti in San Jose
