Question:
Need a sample letter from PCP

I need a sample letter from my PCP. Who does it go to? My surgeon or the insurance company? I am going to see my pcp on Thursday 10/18 and want to give her a sample so it will contain all the pertinent information needed.I am sure this has been asked before but I can't seem to find a sample of the letter. MI never got a letter from my PCP the surgen took care of everything anmy BMI just made 40. Good Luck

my PCP only wrote his name, address and telephone number on a prescription pad with nothing else and said if anyone had questions, they could call him. This is what I gave to my surgeon.

Check out my profile for a copy of my letter from my PCP. Even though many surgeons do not require a letter from the PCP, I believe that the insurance company likes when the PCP is in suppport of WLS. I forwarded my letter to the surgeon who put it in the packet to the ins co.
