I have this insurance and it is great. You only need to have the surgery medically necessary, no supervised anything. You will need to list all the diets you have been on, nothing has to be substantiated or proved. You will need psychological evaluation and one by a nutritionist. I had been precertified for surgery yesterday, but my knee gave out and I had to have it done first. The insurance is wonderful as far as I'm concerned. If you make sure the dr and hospital is a tier 1 provider, you will not have to pay much out of your pocket.
