HELP!! I NEED A LIST OF DIETS THAT I HAVE TRIED BUT FAILED AT FOR SURGEON!

MY FIRST APPOINTMENT TO SEE MY SURGEON, DR. SALAHI, OF KEARNY NEW JERSEY IS SET FOR FEBRUARY 17, 2000. HE HAS ASKED ME TO BRING A LIST OF ALL PRIOR FAILED DIETS. AS MANY OF YOU HAVE HAD TO DO THIS ALREADY WOULD YOU PLEASE TELL ME WHAT FORM YOU USED IF ANY, HOW FAR BACK DID YOU GO, HOW YOU LISTED YOUR FAILURES (OR TEMPORARY SUCCESSES)...I DONT WANT TO HAVE TO GO BACK AND FORTH TO HIS OFFICE WITH REVISED EDITIONS. I HAVE BEEN DIETING SINCE I WAS IN THE 8TH GRADE, SHOULD I GO THAT FAR BACK EVEN IF I DONT REMEMBER ALL THE DETAILS...PLEASE HELP!!! http://www.clos.net/forms/diet_hx01.htm you will find a good form here. I would go back as far as you can remember. I can't remember how much I lost 15 - 20 years ago on the diets, but I listed them all.I wanted them to know this has been a life time battle. Some insurance companys look more at Doctor supervised diets more than anything. Good luck to you. I'm only starting myself. I sent you an email also.

And list ALL diets, including gyms you joined, book and magazine diets, self-imposed fasts and any time your doctor has said, "Lose weight". They're all valid attempts. Be sure to show the wt regain. It only backs up what the National Institutes of Health already says. It's not your failure, it's that diets don't work.
