Question:
Humana six month diet requirement

Make sure he/she documents your weight, any exercise and dieting that you have been doing and the result. Good Luck - Cheryl - surgery date 4/14/06

your wieght cbeck in, your goal for next mounth his councelig to to get there, I suggest that you make a set of stickers with this on it. and take them with you so he can put them in so they boldly stand out. ,you know how they nod and scrible, but if the notation is not there it didnt happen, so a nice lable will pop out on the photo copier, and ask for the copy as u leave, explain that why and what for , My pcp never did it right so my surgions nut did it. it was a pain but we got it done.

You need to take responsibility for what the dr. writes--- say something like "can you read that back to me?" Make sure they document your success as well as your "failures" and if necessary, try a medication or two for one month at a time, change it, modify it, etc. Every little thing is a big help. And, if the writing is bad, get copies yourself and highlight them with a yellow marker before you send the pages to the nsurance company. Any other health issues should be fully documented each visit, even if stable, like hypertension, cholesterol, sleep apnea, depression, etc. Just make sure they doc actually WRITES down EVERYTHING> Cyndi

PS- make sure every visit, the dr. lists your calories and diet recommendation, like "continue 1000 cal, 60 Gm protein, low fat diet". If you can keep a food log, have the dr. write "food log reviewed". The notes need to be very specific. Keep up the good work...your 6 months will be over soon!
