Does anyone know of a indiviual health plan that I can purchase

Here is a small listing of individual health care companies. Keep in mind that insurance companies ARE mandated to offer the surgery, but the employers ARE NOT obligated to purchase the rider. These companies that offer individual insurance ARE mandated to offer the rider. I\'ve had to change from group to individual, I\'m paying the same dollar amount and I\'ll be getting what I want.
