Question:
I need info. or a copy of a letter to send along with my packet for insurance approva

I have noticed on this sight that my carrier for insurance has an 50/50 approval rate and would like to send a letter of plea along with my packet for the surgeons office to send to my Ins. I sent you an email. Hope it helps. I would also call them if I were you. I think by calling it puts a voice with a name.. I called mine well before sending in the letter, and while waiting for my Dr.s office to get the stuff into them.. They told me I got approved before the Dr. office did! Best of luck to you. I sent the mail to the addy on your profile. It says FWD because I just sent it to my mom yesterday too. ~~;-)
