Question:
Anyone with BCBC PPO from Illinois???????????????????????????????

I am looking for criteria for them for WLS. I work for United Airlines and am looking for someone who can give me an idea of what I need to get approved. I have a BMI of 55.7 and I have GERD, as well as back problems and I believe I have sleep apnea but has not been diagnosed by doctor. Any help would be greatly appreciated    — Nicole R. Nicole, I hace BC/BS/ PPO of Illinois throught Ameritech and was approved in 2 weeks. I didn't need to get a referal at all- I went directly to my surgeon. However, it is my understanding that all policies are different and it is up to your employer as to what coverages your particular policy covers, so I have to go along with Ray and call your insurance dept- or BC/BS themselves and see if it is covered. By the way, when I went to my surgeon, I was fully armed with ALL past medical records for co-morbidities, past diets and drugs, etc. and my surgeon sent all that in when he sent in for approval to the insurance- that way, they had everything and the approval process moved along faster. Good Luck!

Call a doctors office to find out if it is covered by your insurance. I have a BCBS PPO from Michigan and I called BTC, which by the way has a hospital in Illinois, and they can told me that my insurance would pay, and it did.
