Has anyone seen this exclusion for UHC Options PPO? Been Approved?

I just had open RNY and I have UHC PPO, all I had to prove was that it was medically necessary.

I have uhc ppo and i called them and because i have co-morbities that are life threating (diabetes and high cholestrol) they said they would approve it. good luck:)
