Unless you have a preexisting condition exclusion (which can only be for the first six months you have insurance), there should be no minimum. If you have insurance through a large group health plan, they have probably negotiated to have no preexisting condition exclusion. If you are changing insurance and have been continously covered for at least six months, the new insurance cannot impose a preexisting condition exclusion. Also, be aware that the definition of preexisint condition means something you have been treating for or consulted a doctor for in the last six months. So, if you have not consulted your doctor or been treated for morbid obesity in the last six months, it cannot be considered preexisting even if a preexisting condition clause applies.

I now have Blue Cross PPO, changed insurances on January 1st, saw the surgeon January 4th, was approved January 31st and had surgery February 12th!
