I was denied from my Insurance company Cigna and I need help appeal letter

Instead of doing the six month diet again, I would appeal the decision, or have your PCP document your six month diet to CIGNA standards and resubmit the package.

I was denied three times and then referred to an outside arbitrator. As soon as it went to them, they questioned Cigna as to why I was denied and they said they never got documentation. I resent the documentation and it was approved. Best of Luck, Robin McLaren
