PLASTIC SURGERY APPROVAL

I had my surgery 7months ago. Could you tell me what you had to do to have your insurance cover it. Was there a time limit for how long after bypass you would have to wait before the surgery? Did you have to be at a maintain weight? Any info would be helpful. I had a lot of excess skin to begin with now It's just longer. Tammy Lowery
