The two most dreaded words- Diet & exercise.  The diet should be low on the glycemic scale (carbs), low saturated fat, LOW calorie and high fiber.  Exercise should be 30 minutes per day, at least 5 days per week.

Insulin.  More specifically, Lantus or Levemir.  Ask your doctor and start as soon as possible.  It's short-term but your best bet for qualifying for surgery.  My surgery got moved up because the lady before me had a glucose level close to 400 mg/dl.  The wouldn't operate on anyone with a glucose level over 300 mg/dl.  She should have been on insulin just to get thru the surgery as scheduled, but had to reschedule everything.  You've come too far to not commit to the best surgical outcome possible.  In my opinion,  that means insulin for optimal control over the next 6 weeks.
