In 1997 Dr Stoner did my gastric surgery.

In 1997 Dr Stoner did my gastric surgery. I was picked up and driven to Iowa for the surgery. Fast forward 26 years later and I'm needing a revision surgery. The new surgeon is needs further information. However, my memory isn't as good as it was back then. Does anyone remember the name of the hospital and the name of what Dr.Stoner's WLS programs was called or who I could contact in the attempt to get what this new surgeon is wanting?!?
