What you need to do is call the customer service number on back of you card and ask them: 1. Is weight loss surgery a covered benefit 2.  What is your criteria for the surgery I know people who have that insurance and it has been covered-BUT depends on your employer.  So give them a call and have them mail you the criteria so you have it in writing! Best of luck!! Shannon

As of January 1, 2008, the rules of Blue Cross of Michigan will change significantly.  No need for preauthorization.  If your BMI is greater than 50, the diet/exercise program is waived.  If your BMI is less than 50 you will still need the diet/exercise program but it has been reduced from 12 to 6 months.
