NEED A COPY OF SIGN IN SHEET ASAP??

Hello everybody! I just talked to Dr. Scott's office and the only thing they need is a copy faxed to them of the sign in sheet from the meeting I came to in Springfield last month. I have no idea who to call to get this. Can anybody help me asap? As soon as they get it they can set my classes and then a surgery date. Yeaaaaaaaaaaaa!!! Thanks for any help!!
