HELP I need to find a psych evaluator

I need to have a psych eval done as my final step so my insurance will approve my surgery.  Does anybody know of one or maybe know a doc that can do a consult over the phone?

i dont know of any of them that do a psych consult over the phone, there may be but i have never heard of one...
