My DH's PCP messed up the Documentation of the Medically Supv Weight Loss

It is VERY frustrating!  The same thing happened for 2 of my 6 visits and I had to repeat those visits.  I was very nitpicky and told them each time I was there what I needed, too, but they missed it twice.  Fortunately, I didn't have to repeat the entier 6 months.  I know he must be irritated!

Hi Dena,

Thanks for the commiseration! It's really unfortunate that there isn't a clear understanding of what's required. I know that our Dr's office regularly refers patients to Duke for WLS and so I have say that I was absolutely shocked by the lack of proper annotation of the clinical records. They got it right on on the 6th month, so he only has to redo 5 months ... one of which he did this month. That means he has 4 more to go before we can resubmit.

We'll be keeping a close eye on things from here on out.

Hi Barb

I had this happen to me as well.  Nothing like doing the 6 month diet twice and then having to repeat some of those preop tests   But on the positive side, I was really psyched for surgery!  It gave me more time to sit on these boards and read and learn, and deal with my nervousness.  by the time surgery rolled around I was super ready!  Just make sure hubby does not leave the pcp's office at each visit until they have recorded EVERYTHING.  I made sure at each visit that it was documented the second time around so it was not forgotten.  (((((((HUGS))))))) to you and your hubby.

I have to tell you that my heart sunk when I saw my husband Friday evening and he told me that he'd seen the PCP that morning. UGH! He made sure that they talked about diet and exercise, but he dropped the ball on the behavioral management issues. I told him that I think I'm going to give him a check list that he'll need to keep in his wallet to ensure that he covers all of the bases. I hope that this won't be a wasted month, but we'll just have to wait and see.

I wanted to see if I could go with on the next appoinntment, but I'll be in CA when that one rolls around, so I guess we'll just have to give him the pep talk, check list and a prayer. I'm much more pro-active about this stuff. I actually tell them that I need to have this and that documented. He seems to expect that they are seasoned professionals and that they'll do what they need to do ... and to be honest, they do for the most part, but this nit-pickey documentation thing is what concerns me. I'll probably call the office and see about picking up a copy of the clinical notes so that I can see if "I" think that they'll pass muster and then supply a copy to the folks who handle the insurance submission at Duke to see if we have their blessing as well. We don't want to have to do this for a 3rd time.
