While Sr Sudan certainly performs DS, according to my own knowledge he will NOT revise RNY to DS.  He goes the route of ERNY as an RNY revision.  He will only revise to DS from the band and even then only in the best of cir****tances.

Obtain a copy of the surgical report of your revision.  This will make clear what was performed.  If you don't understand it, post here or have a Dr explain it in layman's terms.  Heck, even fax it to Duke and have Sudan call you and talk to you about it. I'm pretty sure he will talk to his patients from NE.  I have all the phone numbers if you need them.

You need to at least CALL the nutritionist at Creighton and tell them what has been going on and ask for guidance.

Phone calls are free!  :)

If your symptoms are as bad as you say it is, try being honest about what you are eating and take measures to correct any bad habits. (I'm working on that myself right now)  There is LOADS of good advice on this subject right here on this board.  You may have severe food intolerances that have not been addressed.  It is common to have intolerances to dairy, gluten and sugar alcohols amongst other things.  If you've resorted to wearing diapers, you seriously need to take a look at what you are eating.  If you are ERNY not only do you have a great deal of malabsorption, you have no pyloric valve to properly regulate food passing into your intestines.

You also MUST have a FULL set of lab work done ASAP.  That is not an option.

Do you take any vitamins?

You must start advocating for yourself now.

Coming to this board is a great first step :)  The knowledgeable people here aren't going to sugar coat things, they'll be honest with you and that's a good thing.

Wink,
There is a lot of information for you to absorb in this thread. At the very top of the page you will see a link titled " Bookmark " If you click on it, it will allow you to save this thread and you won't have to go looking all over the place for it in the future.

Good luck

Some good advice from another poster:

I have one suggestion in regards to getting the O.R. report. She should call the hospital where her surgery was performed and get her O.R.,labs and endoscopy report(s) from them.

Let me add or amplify on a few things to this parade of information:

1) With a distal RNY or ERNY (which are both a gastric pouch together with a similar but not the same intestinal RNY construction, but both having a significant amount of intestine bypassed), you are in some ways more like a DSer than an RNYer - but (and I realize you aren't going to want to hear this) you have the worst of both surgeries.  You have a stoma and thus no pyloric valve, and you have a lot of malabsorption.  And no duodenum in the alimentary tract.  I can't believe anyone still considers these surgeries as "standard of care" but they do.  You are going to have to work VERY VERY hard to live healthfully with this mess.

2) Fortunately, the most vitamin knowledgeable person on these boards is Michelle, AKA Vitalady, to whom you can write (I won't give you her email here, but you can find her at www.vitalady.com), who has a business selling vitamins to bariatric patients, AND is an unbelievable source of information and just plain caring.  Now, you might get suspicious about someone who touts vitamins on these boards, but trust me (and the hundreds if not thousands here who use her services), she is the real deal, and she pays to advertise here too.  And she is a long-term distal RNYer.  Once you get your labs done, she will be of great help in getting your nutrition back on track - and I am SURE you are in some serious trouble already.  She is the ONLY reasonable source (and it is reasonable) for the Vitamin A, D and K (probably E too) that is in the "dry" form and in the huge dosages that those of us who have distal bypasses (DS and distal/ERNY) need and can absorb.

3) You not only are likely to be deficient in a number of nutrients, which is likely to be contributing to your issues, but you are UNDOUBTEDLY consuming a large number of things you shouldn't be putting in your mouth.  CUT THESE THINGS OUT!!  Starting with white flour (no bread, pasta, cookies, cake, NOTHING with flour in it, and for now, including whole grain flour), sugar, tomatoes (for now), dairy and (for now) greasy stuff including fried food, until you get your bowels under control.  No sweeteners that end it "itol" either (sorbitol, maltitol, xylitol, e.g.).  They are LAXATIVES.

4) For starters, as Beemer said, you need to IMMEDIATELY start taking calcium CITRATE (be CAREFUL - the brand names are confusing - Citrical is calcium citrate; Caltrate is calium carbonate and WORTHLESS) - start with 2 pills at a time (they should be HORSE PILLS - don't get the Citrical Peitites, or if you do, take 3 of them at a time - and if you read the label, it will ALWAYS require at least 2 pills to add up to one dose of 500-630 mg of elemental calcium) at least 4 times/day.  You are undoubtedly low on calcium - BUT it is not your serum calcium that will tell that tale, it is your PTH, which MUST be tested as well (and you should probably get a DEXA scan immediately as well to see how bad your osteoporosis already is).  PTH being high indicates that your body is robbing your bones and teeth to keep your serum calcium stable (if your serum calcium goes too low, your HEART STOPS!).  And start taking some kind of iron supplement - not only are you almost certainly low, but both of these supplements are constipating and could help firm up your poop.

5) Buy an OTC prenatal vitamin like Prenavite (you may have to get the bottle from the pharmacist - my Walgreens doesn't leave them out, although they do not require a prescription).  It has some basic stuff in it at somewhat high doses.  Yes, men and women our age (I'll be 58 next month) buy prenatals.

6) Chia seeds - get them from a health food store and start taking them.  I don't know how much or how to get them in (ask Chris Michaels on the DS forum), but I have heard nothing but great things about them for firming up loose stools.

7) Probiotics - if you have a bacterial overgrowth (likely), you will need these to help fix it, even before and if you end up on antibiotics eventually.  Get the good kind, including something with S. boulardii like Primal Defense Ultra - it can be purchased in stores, but it is significantly cheaper on the internet.  Most of us with distal bypasses take them as part of our daily regimen, just to keep our guts populated with the right bugs.

This is going to take some effort, consistency and patience on your part, but this CAN be fixed.  Once you get everything under control and back on track, it won't be so hard to keep it there, especially once you know what you should and shouldn't be eating.

BUT YOU HAVE TO DO YOUR PART, WHI*****LUDES BEING DILIGENT ABOUT YOUR HEALTH, NO MORE EXCUSES!!

I too find it hard to believe that someone with IT knowledge and access to a public library would have allowed herself to get into this sorry state - but you have taken the first steps to getting your health and your life back.  Now you must follow through.

Until you start getting a handle on your particular troubles, I'd suggest sticking to lean meats, eggs, non-starchy veggies. Not very interesting, I know, but things WILL get better.

As Ms. Batt said.  Eat protein, but stay away from fatty meats for now until things settle down.  Get a rotisserie chicken; eggs, fish (but not escolar!), deli turkey, baked or boiled potato, oatmeal, banana, edamame, yoghurt but only if you're SURE dairy/lactose isn't a problem, nuts (not cashews), chicken soup (no noodles or rice), and maybe some protein drinks/shakes to get more protein in.

Here is a thread from a couple of years ago - notice that Joyce eventually ended up revising to a DS, because she couldn't deal with the malnutrition and ****ting: http://www.obesityhelp.com/forums/revision/3981178/Help-ERNY S-or-anyone-with-similiar-issues/

But your best best is to talk to Michelle.
