I suggest cross-posting this to the Insurance forum, and the CA forum. Also, contact Diana Cox---she's had a lot of experience with insurance, especially in CA.

Also, look into having your DS outside the US. There are several excellent surgeons available, at about HALF the cost of having it done inside the US---and that includes air fare, etc.
