Yeah, that's three calendar months, I think, not four.  Well, not even three.

4/23 to 5/15 is less than one month.

5/15 to 6/13 is one month, so that's almost two months, but not quite.

6/13 to 7/11 is one month, so that's almost three months but not quite.

You would need at least one more visit.  Probably two more, if four months is required.

I know every insurance company is different, but my supervised weight loss visits had to occur at least 30 days apart. You might want to check with your insurance company to clarify what they require.

I think I have decided to just do it through my surgeons office. My insurance will count that as being my 6 month weight loss. It just means I have to do the program two times instead of one since it is only a 12 week program. At least their I know my surgeon and my insurance company will be happy. I might get stuck paying for some of it but I was willing to go to Mexico so I guess paying for this isn't that much of a big deal. My first insurance company will pay one amount and my second insurance should pick up the rest ( I hope ). I might even do my PCP just to be double safe like one person told me she had done. Either way it something I have to do to get the surgery and I think just talking it out with everyone has helped so thanks to all that replied.

I think it's smart to do your PCP visits too. I hope it all works out well for you :)  

Sorry about asking questions on your post, but I'm confused, as well.

I think when it comes to insurance companies it is so easy to get confused. I also think they count on that so they can make more money. Oh well like my mother use to say it will all work out.

Twice a week for 12 weeks (and then doing that twice) seems excessive to me - that's 48 visits.    The six month program for my insurance consisted of 7 visits with a nutritionist spaced 1 month apart.    My surgeon, PCP, nutritionist all work at the same place (Virginia Mason Medical Center).    7 visits with a doctor or nutritionist, spaced 1 month apart seems to be pretty standard from what I've read here.
