Need to find a good Primary care Physician in the West Valley

I am having Gastric Bypass surgery on August 14th and I need to find a new primary care physician. I live in Avondale, AZ. I am unhappy with my current doctor. Can anyone help?

Hey shanrenee,

I have a great PCP, but it's somewhat north of where you are. My PCP is Dr Thomas Mertins at Union Hills Family Medicine. His office is on 83rd Ave south of Union Hills Rd. I moved to downtown Phoenix 2 years ago and still kept him as a PCP! He is that good! Their phone number is 623-583-0306.

Can someone please post a list of items needed at the hospital? Also I am having surgery at St.Lukes in Phoenix with Dr Juarez, does anyone know if the patient rooms are private?
