Anyone have to fill out the Family and Medical Leave Act Forms?

I used it too, I work for the government and they can't hold using the sick leave against you for promotions or transfer.

Do you have to fill this out if you are going out on disability for your surgery?

Are you referring to short term disability? Most employers run STD concurrently with FMLA. FMLA is unpaid but guarantees your position or a similar one with the company. STD is generally paid.
